Note : वैसे उम्मीदवार जिन्होंने एक साल या उससे अधिक कही ट्रेनिंग अथवा एक्सपीरियंस लिया हो वैसे उम्मीदवार फॉर्म भरने के योग्य नही है |

- self attested copies Age Proof, Education and Technical Qualifications
- Domicile Certificate Copy
- A character certificate issued by the Gazetted Officer. (Original)

- One passport Size Photo चिपकाना है
- One self addressed envelopes (size 10″ x 8″) affixed with Rs. 45/- Postal stamp.
