Western Railway की apprentice आई हुई थी जिसकी विडियो मैंने अपलोड किया था | 2 महीने से इसकी सभी जगहों की document verification चल रहा है | फ़िलहाल Western Railway Ratlam Division का 2 बार document verification हो चूका है | अब तीसरी merit list जारी किया गया है | इसमें कुल 325 candidates select हुए हैं | जिनका document verification 20-10-2021 से शुरू होने वाला है | जिनका निचे दिए गये list में नाम होता है उनको document verification के लिए दिए गये स्थान पर समय से सही dress में आपको जाना होगा |

आपको सूचित किया जाता है कि दस्तावेज़ सत्यापन सूची में आवंटित तिथि पर दस्तावेज़ सत्यापन के लिए आपको ईएम सेक्शन 2nd फ्लोर, डीआरएम कार्यालय रतलाम (डब्ल्यूआर) पिन -457001 में भाग लेना होगा।

आपको मेडिकल फिटनेस सर्टिफिकेट के साथ सेल्फ अटेस्टेड ज़ेरॉक्स कॉपी (आवेदन पत्र और अपलोड किए गए दस्तावेज़) के 2-सेट के साथ मूल दस्तावेज़ लाना होगा, सरकारी अधिकृत डॉक्टर (गज़) द्वारा हस्ताक्षरित होना चाहिए, जो कि सहायक के पद से नीचे नहीं है। अनुलग्नक “H” के अनुसार केंद्रीय/राज्य अस्पताल के सर्जन।

INSTRUCTION  FOR  CANDIDATES  ATTENDING   DOCUMENT  VERIFICATION

• Ø  Candidate must bring the following Document / Certificate in original along with two sets of photocopies    of all the documents duly self attested. 
• Ø  ·Print out of Application Form
• Ø  SSC (Standard 10th ) or equivalent Mark Sheet
• Ø  ·Date of Birth certificate (Standard 10th or its equivalent certificate or mark sheet indicating date of birth or School leaving certificate indicating date of birth.
• Ø Consolidated ITI marks sheet for all semesters of the Trade in which applied / Provisional National Trade Certificate indicating marks.
• Ø National trade certificate issued by NCVT or Provisional National Trade Certificate issued by NCVT / SCVT.
• Ø Caste certificate for SC/ST Candidate as per Annexure ”B” /  OBC candidate as per Annexure “C” & “D” in the prescribed format issued by Competent Authority. OBC Cast certificate should have  clear  mention about non Creamy Layer status valid for current year 2020-2021.
• Ø  You are also advised to apply for latest OBC certificate with Non Creamy Layer certificate valid for the current year 2021-22 so that it can be available for submission at the time of document verification.
• Ø Discharge certificate or Armed Forces services certificate in case applied against Ex-Servicemen (Ex-SM) Quota.
• Ø Disability certificate for PH candidate as per Annexure “E”&“F” .
• Ø Four identical passport size photographs as per scanned photo in Application form.
• Ø Medical fitness certificate Should be signed by Government authorized doctor (Gaz.),  not below the rank of Asst. Surgeon of Central/State Hospital as per Annexure “H”.   
• Ø Aadhar Card / Voter ID / Passport / Driving License or any other valid Governments Identity  Card.
• Ø  Attend the Document Verification center well in time with valid documents as asked for.
• Ø  No request for change of center / date of Document Verification / time will be entertained.
• Ø  Candidates will have to bear all expenses with respect to stay, Transportation etc. They may also require staying for more than one day in case of any requirement/exigencies. Accordingly they should come well prepared to stay at Document Verification center for One or TWO days.
• Ø  Candidates are directed to ensure that certificates produced by them do not have any deficiency with  respect to name, father name, date of birth, mark etc. Failing which their candidature is liable to be cancelled.
• Ø   Violation  of  these instructions shall lead to cancellation of candidature.
